If you are an eligible member living with congestive heart failure, join the Congestive Heart Failure Education Program.

If you join, free information will arrive in your mailbox monthly. No classes to attend, just solid information including:

  • Complete congestive heart failure care booklet
  • How your body reacts to heart failure
  • Heart failure care goals and records
  • Exercise and activity plans
  • How to follow a low-sodium diet
  • Keeping up with your medicines
  • Warning signs of a flare-up
  • A registered nurse to answer questions or assist you

  • Managing feelings about heart failure
  • Smoking cessation resources
  • Tips for talking to your doctor
  • Web and telephone resources

Here's how to join and get started on a plan for a healthier you!

Download the member enrollment form [pdf] and return it in one of four ways:

  1. Fax to 501-378-6693
  2. Scan and email to HealthEducationTeam
  3. Mail to:

  4. You also can call your nearest office location and ask for a Registered Nurse case manager to discuss the program and receive assistance in enrolling in the Congestive Heart Failure education program.
