Includes access to an in-state provider network and applies to the following plans: HA Platinum Standardized, HA Silver AH (ARHOME), HA Silver Standardized, HA Bronze Exp Standardized, and HA Gold Standardized plans.
Member ID cards do not have “PPO” in the bottom right.
