1095B tax forms will not be mailed in 2021

Under the Tax Cut and Jobs Act, Americans no longer need to verify proof of insurance coverage when they file their calendar year 2020 federal tax return. Due to this change, 1095B forms will not be mailed in 2021. If you still want a 1095B, you can access it through the My Blueprint member portal beginning March 2.

  • Send a letter to:
    Health Advantage
    P.O. Box 8069
    Little Rock, AR 72203-8069

Under the new law, Health Advantage must furnish a copy of a 1095B to the responsible individual within 30 days of receiving the request.
